A committee comprising of mental health professionals meets as and when admission and discharge is needed. The committee assess clinical condition and decides on admission and discharge. Every 3 months, progress is monitored. If needed, the rehabilitation plan is also altered based on the outcome of the review. Caregivers are briefed about the status of their child. All the mental health professionals are involved in this exercise. If the resident is ready for discharge, discharge protocol is followed.

Follow up is an important component in the rehabilitation programme of the Trust. At the time of discharge, caregivers are given orientation about the need and importance of follow up and in consultation with them, a follow up schedule is drawn by the professionals. Follow up remainders are sent before the scheduled date and if a person fails to come for follow up for two consecutive days, key worker makes a home visit to interact with family members and to provide necessary support that will ensure follow up.

| The clinical assessment is done by a team consisting of a clinical psychologist, the psychiatrist and the medical officer. The family assessment is done by a social worker, Based on the findings, the persons will be hospitalized for treatment. The various units having residential facilities at the foundation carry out these activities. Nearly a number of 40 and above, trainees, engage themselves in actives such as sowing, ploughing, irrigating, manuring and harvesting the agricultural crop.The trainees are allocated a portion of land marked in their name. This has a therapeutic bearing where in the trainee develops an affinity of owning the land and there by works with a sense of confidence and dignity. The yield is distributed for consumption amidst the various units of the foundation and the rest sent for public marketing. Animal Husbandry activities: Persons with psychiatric disabilities are found to have a special emotional bondage with pet animals. Our experiences eventually led to the developing of a animal farm that houses cows, dogs, gunniea pigs, rabbits, poultry, doves, love birds and fishes. Almost cent percent of the trainees enjoy touching, feeding and playing with the animals and participate in looking after the animals. This plays a therapeutic role in the developing of accountability in the execution of the various activities in care giving of the animals. Gokulam- Dairy activity: Gokulam established in the year 1995 at Shristi.This diary unit has 50 milch animals and offers training in multi various Diary activities such as feeding, milking and cleaning the cows to nearly 15 trainees per batch for a period of six months .Nearly 500 liters of milk is procured per day which is made available for the self sustenance of the various units of the foundation and the balance amount is given for commercial marketing by the trainees themselves. Out of the 300 trainees who have undergone vocational rehabilitation in a year nearly 25% of them are able to get placements. The hallmark of the placement wing is that the professional staff is in constant interaction with the employers and monitors the developments of the trainees and offer guidance and counseling as required by the situation. The trainees in placement meet the therapists once a month during the meeting held for sharing and interacting about their experiences in working. They also go for tours and picnics once a year for a get together. Research: The department of Science & technology of the Government of India recognized the Foundation for their outstanding involvement and work in the field of mental health & Rehabilitation in the year 1995.The result of the recognition brought about the government supporting the foundation in the field of mental health research. This enabled the foundation to develop inbuilt research in the various programmes carried out in the field of psycho social rehabilitation.
The initial periods saw the research carried out regarding mental health issues in educational institutions, industries, private and public sectors covering a target group of teachers, students, lecturers, labours, industrialists and the general public portraying the themes such as stress, anxiety, drug and alcohol, memory and concentration, absentism…
What gave raise to the formulation of awareness programmes as the result of the study paved ways for data oriented field based research work to be presented as research papers in national and international level seminars, workshops and conferences.
Still further experimental research was under taken on evaluating psycho social intervention programmes and administering certain psychiatric drugs. The core activities of the research and development wing are: - Establishment of a document centre.
